Direct Billing

Jacquelyn Fleming is part of the Independent Practice Network and is able to direct bill to over 30 different insurance carriers. Please be aware that if your insurance does not cover the full balance of the services provided that you are responsible for paying the remainder at the time of services rendered.

Privacy

Your privacy is very important and we take it very seriously. All personal health information that is collected by Beaumont Dental Hygiene Studio is collected, used and disclosed in accordance with the provisions of the Health Information Act (HIA) and is primarily used to provide diagnostic, treatment and care services to you. The privacy provisions of the legislation require that we protect your health information against unauthorized access, use disclosure or destruction.
